AAC Report Coach
A specialized template to help Speech-Language Pathologists (SLPs) quickly prepare complete funding reports for SGD recommendations.
What is the AAC Report Coach?
The AAC Report Coach is a Microsoft Word template designed by Pam Mathy, Ph.D. It guides SLPs through the process of writing a funding report that meets the rigorous requirements of Medicare and other major funding sources.
Directions for Use
- Download and Open: Save the
AAC Report Coach.docfile to your computer. - Save as New: Rename the file for your specific client (e.g., "SGD Assessment Report - Client Name").
- Enable Forms: Select the Lock icon from the Forms toolbar (View > Toolbars > Forms).
- Navigate with Tab: Use the Tab key to move between fields. Gray boxes indicate where information can be entered.
- Review and Edit: Once the fields are filled, unlock the document to make final edits, ensuring all client-specific details are accurate.
Important Note for SLPs
The AAC Report Coach should be used after a complete SLP and AAC assessment has been conducted, following Medicare protocols. Much of the information is included because it is specifically required by funding sources—be cautious when deleting sections.
